enrollment requirements

Thanks for your interest in our school. Should you choose to enroll your child at Get Smart Kids Academy you will need:

To complete an enrollment packet
To submit updated blue and yellow cards from your child’s pediatrician
TB vaccination updated
A recent picture of your child
A copy of their birth certificate and social security card
Copy of parent(s) identification and social security #
To submit payment of registration/supply fees and the first week’s tuition.

On your child’s first day they should bring a backpack with a change of clothes, 2 blankets for rest time and diapers/wipes (if applicable). Please label everything with your child’s name. *Other supplies may be required by age group.
